Online Appointment Booking Guide

By using the above booking tool you are committing your appointment to our schedule. If you need to make a change to your appointment after securing your appointment date and time, please contact the office as soon as possible. Note that we require more than 48 hours’ notice to reschedule or cancel your appointment.

If you have not been seen by our practice in the last 3 years Federal Regulations require you to schedule as a “New Patient”. Choose one of the “New Patient” options under Patient Type when booking.

New patients must complete registration within our Patient Portal. You will receive a text and/or email with a link to register for our patient portal immediately after booking your appointment. Follow the in-app guide to complete your medical history and consent forms at least 48 hours prior to your appointment. Incomplete registration may result in cancellation of your appointment. If you do not receive a link to register or have trouble accessing the patient portal please contact our office at .

If you are scheduling for a TeleHealth visit you will use the booking confirmation link, sent via email and/or text message, to connect to your virtual visit at your scheduled time. It’s best to use a device with audio and video capabilities for your TeleHealth visit. TeleHealth appointments are available to residents of Nebraska, Nevada, Tennessee and Hawaii only. Please note that all available appointments are in Central Standard Time (CST).

Insurance Coverage:

Please be sure to contact your health insurance provider to ensure that we are In-Network with your plan and that your plan has Telehealth coverage, if applicable. As the world of TeleHealth evolves so does coverage for TeleHealth services so be sure to consult your health insurance benefits. At the time of booking your visit we will pre-collect the visit fee. When your insurance claim is processed and any amount is paid by your plan resulting in an overpayment you will be reimbursed appropriately.


We require 48 hours' cancellation notice if you are unable to keep your appointment. If you do not give sufficient cancellation notice or if you no-show your appointment you will forfeit the pre-collected visit fee or will be charged the visit fee appropriate to your appointment. These fees are non-refundable.
